There are 2 ways to get the completed forms to us:

Please see the Registration Page for information indicating when Preschool Applications will be accepted.

IN PERSON- Open the form and complete it in your web browser. Print it and sign where required, then hand it to the preschool during the designated registration hours.

VIA EMAIL- Open the form and save it on your computer. Complete the form with Adobe Reader and use a digital signature where parent signature is required. You can then save the form and send it to Miss Jenn via email. Please make sure you receive a confirmation email so that you are sure the form was received and accepted.
